Eastern Airlines Flight 401 crashed into the Florida Everglades while on approach to Miami International Airport. The National Transportation Safety Board (NTSB) determined that the crash was the result of an inadvertent autopilot disconnection that went unnoticed by the flightcrew as they were attempting to correct an unsafe landing gear position indication. The NTSB determined that the uncommanded descent into the Everglades was the result of the flightcrew’s failure to monitor the airplane’s flight path and an improper division of duties on the flight deck while troubleshooting an anomalous system indication. Of the 163 persons on board 112 were killed in the crash. This accident was one of the precipitating accidents leading to the development and industry-wide adoption of flightcrew resource management philosophies and training.

This three-dimensional (3-D) animated reconstruction shows the last 2 minutes of the February 12, 2009, accident involving a Bombardier DHC-8-400, N200WQ, operated by of Colgan Air, Inc., which crashed about 5 nautical miles northeast of Buffalo-Niagara International Airport, Buffalo, New York, while on an instrument landing system approach to runway 23. During the approach, a pitchup motion occurred, followed by a left roll and then a right roll. During these maneuvers, both the stick shaker and stick pusher were activated, and the speed decreased. After further pitch and roll excursions, the airplane entered a steep descent from which it did not recover.
The animation shows excerpts from the flight data recorder (FDR), the pit voice recorder (CVR) transcript, recorded radar data, and aircraft performance data. It does not depict the weather or visibility conditions at the time of the accident. The animation does not include audio.
The upper portion of the animation shows a 3-D model of the airplane and the airplanes motions during the accident sequence. In this area, selected content from the CVR transcript or other annotations are superimposed as text at the time that the event occurred. All times (in eastern standard time) are shown on the right side of the screen.
The lower portion of the animation depicts instruments and indicators, which display selected FDR or calculated parameters. The instruments and indications are shown in three sections, which are (from left to right):
•Airspeed, airspeed tape, low speed cue, attitude indicator showing pitch and roll attitude, altitude, altitude tape, rate of climb, and heading;
•Stick shaker and stick pusher indicated as text, control wheel/column icon depicting the control wheel (rotating right or left) and control column (moving up or down) inputs, and an indicator showing rudder pedal inputs; and

NEW YORK (WABC) — A photo shoot involving an Air Force aircraft and a fighter jet led to hundreds of frightened calls from residents and workers in Lower Manhattan Monday.

For a half-hour, the Boeing 747 and F-16 jet circled the Statue of Liberty and the lower Manhattan skyline near the World Trade Center site. Offices evacuated. Dispatchers were inundated with calls. Witnesses thought the planes were flying dangerously low. The exercise involved an F-16 escorting the plane, a Boeing 747, over Battery Park City and up the Hudson River. Some reports quoted an administration official who said the purpose of the photo op was to update file photos of the president’s plane near the Lady Liberty.

The large plane is the military version of the 747 and is the type of plane the president uses to fly. When he is aboard, the plane is referred to as Air Force One.

The head of White House Military Office apologized for the incident late Monday.

“Last week, I approved a mission over New York. I take responsibility for that decision,” said Louis Caledra. “While federal authorities took the proper steps to notify state and local authorities in New York and New Jersey, it’s clear that the mission created confusion and disruption. I apologize and take responsibility for any distress that flight caused.”

President Obama was apparently not informed of the flight. When he learned what happened, sources said Obama was furious.

The maneuver was coordinated in advance with the FAA and state and local officials.

The Defense Department “coordinated the flight with FAA,” FAA spokeman Jim Peters said, “and we made the notifications to the city of New York, as well as the 311 and 911 centers, the NYPD, New Jersey State Police and various operations centers in the New York area.”

City officials were notified, but not the general public.

“The flight of a VC-25 aircraft and F-16 fighters this morning was authorized by the FAA for the vicinity of the Statue of Liberty with directives to local authorities not to disclose information about it, but to direct any inquiries to the FAA Air Traffic Security Coordinator,” a police statement read.

New York City Mayor Michael Bloomberg says he is furious that the federal government flew the planes near Ground Zero. Bloomberg says the flyover so near the World Trade Center site showed “poor judgment” and was insensitive. He says he is furious that the NYPD and another city agency were notified last week, but did not tell him.

If he had known, Bloomberg says he would have tried to stop it.

Flight 1549 Plane Crash Hero Pilot Chesley B. “Sully” Sullenberger Airbus 320

This is a Genuine G-Shot prepared in tribute to a true hero: Captain “Sully” Sullenberger, the Airbus 320 pilot who made the remarkable emergency miracle water landing yesterday of US Airway Flight 1549 on New York’s Hudson River in New York City.

Experience does indeed count, I am about the same age as Capt. Sully, with 27 years experience as a San Francisco 911 police, fire and medical dispatchers with the City and County of SF Department of Emergency Management. I can also attest to the value of training and experience and the need to keep a cool head as so many lives depends upon our every move when seconds really do count.

Well done Captain Sully, we all have reason to be proud of you as a real American hero.

NEW YORK (AP) — A cool-headed pilot maneuvered his crippled jetliner over New York City and ditched it in the frigid Hudson River on Thursday; all 155 on board were pulled to safety as the plane slowly sank. It was,As New York Governor Patterso said, “a miracle on the Hudson.”

One victim suffered two broken legs, a paramedic said, but there were no other reports of serious injuries, just lots of wet socks, Capt. Sully walked the entire length of the sinking aircraft twice to make sures all souls had safely exited the plane.

US Airways Flight 1549, an Airbus A320 bound for Charlotte, N.C., struck a flock of birds after takeoff just minutes earlier at LaGuardia Airport, apparently disabling the both engines.

The pilot, identified as Chesley B. “Sully” Sullenberger III of Danville, Calif., “was phenomenal,” passenger Joe Hart said. “He landed it — I tell you what, the impact wasn’t a whole lot more than a rear-end (collision). It threw you into the seat ahead of you.

“Both engines cut out and he actually floated it into the river,” he said.
No doubt due to his 40 years experience as a glider pilot.

Capt. Sully thank you for a job well done!

“As an amateur astronomer of nearly 20 years experience, this is the second time I have seen something in the sky that is difficult to explain. I was on my apartment stoop smoking a cigarette when my neighbor, who was just coming home after parking his car in our complexs lot, called my attention to something around the corner of the building. I moved to share his line of sight and immediately saw an equilateral triangle composed of six red lights. The lights shone at approximately -2.5 magnitude and covered an area of roughly 9 square degrees in the sky. Three lights marked each of the triangles corners, and three others were arranged outside around them, forming a larger triangle, equal in proportion and orientation to the inner one. As I watched, the individual points were in motion with respect to each other, but the group appeared to hang motionless in the southwest sky. At this point, the approximate alt-az of the lights was 160°(SSE) by 35° elevation.

I ran inside and retrieved my 6×30 image stabilized binoculars to get a better look. The formation had shifted to an irregular lambda shape of five or six lights when I came back outside. Through the binoculars, the lights appeared as tiny points against a backdrop of clouds illuminated by nearby Morristown. They were clearly below the cloud deck compared to airplanes elsewhere in the sky at the time, and appeared to be separate objects in no way connected by a larger structure. They moved steadily in ascending, lateral, and descending directions in a manner that seemed to display deliberate coordination, but there was no noticeable sound of aircraft.

I returned inside again and readied my digital camera after waking my fiancé. By the time I started collecting video, the visible lights had been reduced to three or four and had moved to approximately 130°(SE) by 20° elevation and the formation covered less than the area of the full moon. By this time, eight minutes had passed from when I originally observed the lights. My fiancé and I watched them, with two of my neighbors, as they hung low in the southeast sky, flickering in a tiny triangle. They shifted into a straight line before the individual lights began to rapidly descend beyond tree line one by one. One or two could still be seen hovering near the horizon in the distance before they vanished, leaving only a solitary beacon remaining higher up until it abruptly faded.

If the lights had not ascended during this event, I would have been fairly certain they were some kind of aviation flares, but I cannot confirm, as I am not familiar with that sort of thing.”
